What to include
A valid notice should identify the copyrighted work, the material you claim is infringing and where it appears, your contact information, a good-faith statement that the use is not authorized, and a statement under penalty of perjury that you are the owner or authorized to act, plus a physical or electronic signature.

Counter-notice and repeat infringement
We may forward notices to the subscriber who posted the material. We may terminate accounts of repeat infringers where appropriate.
